How your answers are used
The short version sits beside the checkbox on the survey itself. This is the same thing said slowly, plus the parts that did not fit.

The short version
- Your answers may be published on this site, anonymously — quoted or counted.
- They may also be used commercially: in aggregate statistics, derived datasets, research and data products — and may be licensed or supplied to other organisations, always anonymised or aggregated, never as an identifiable record of you.
- You give Career Field Notes a perpetual, irrevocable, worldwide, royalty-free licence to use, reproduce, adapt and publish your answers for those purposes. You keep ownership of what you wrote.
- You confirm the answers are your own, and that you have not included anything identifying a patient, an employer, or another person.
- Taking part is voluntary, every question is optional, and there is no payment.
- We never ask for your name, date of birth, employer or address. Email is optional and is stored separately from your answers.
- Nothing is published automatically — a person reviews every response first, and you can ask us to remove yours at any time.
What is collected
36 questions, every one of them optional, plus three things the form records about the submission itself: when you started, how long you took, and whether anything about it looked automated. Those three exist so a human reviewer can tell a considered answer from a bot, and for no other purpose.
We never ask for your name, date of birth, exact age, home address, employer's name, or anything about a patient. If you type any of those into a free-text box by accident, the review step is there to catch it — but please don't.
Email is optional and separate. If you give one it is used to send you the results or to reply to you, and it is kept apart from the response itself so that the answer set carries no contact details. Joining the email list is its own tick-box — “Email me when the results are published (about once a month, no marketing).” — and leaving it unticked changes nothing about your response being accepted.
What we may do with it
Publish it on this site, anonymously: as a quotation, or counted into a figure. Use it commercially — this project intends to pay for itself, and the answers are part of what would make it worth paying for. That includes aggregate statistics, derived datasets, research write-ups and data products, and it includes licensing or supplying those to other organisations.
Anything shared outside this site is anonymised or aggregated. We do not sell, license or hand over an identifiable record of you, because we do not hold one: there is no name attached to a response, and the optional email is stored somewhere else.
In licence terms, you give Career Field Notes a perpetual, irrevocable, worldwide, royalty-free licence to use, reproduce, adapt and publish what you submitted, for those purposes. You keep ownership of what you wrote; this is permission, not a transfer.
What you confirm
That you are 18 or over, that the answers are your own experience, and that you have not included confidential information or details that identify a patient, an employer, or another person.
No payment
Taking part is voluntary and unpaid. There is no prize draw and no compensation. The return is that the results become public — the thing that did not exist when you were deciding whether to do this job.
Nothing is published automatically
Every response lands in a review queue and stays there until a person reads it. Below 100 approved responses the site publishes no aggregate figure from this survey at all — only individual approved quotes, labelled preliminary — because a percentage drawn from a handful of self-selected volunteers is noise wearing a lab coat.
Removing your response
Ask, and it goes. Use the removal form and describe which response was yours — roughly when you sent it and something you wrote is usually enough, since we hold no name to look you up by. Removal takes it out of the response set and out of anything derived from it going forward. Aggregate figures already published elsewhere cannot be unpublished, which is the honest limit of what a promise like this can mean.
If this wording changes
Every response is stored with the version of this text that was on screen when it was submitted, so what you agreed to is a matter of record rather than of memory. A later version never applies backwards to a response given under an earlier one.
